The fire service’s greatest health risk: PGI Safety’s case for treating cancer as operational risk

Firefighter cancer risk has shifted from emerging concern to operational reality, driven by clinical evidence, national mortality data and lived experience across the service.

While awareness has grown, many brigades still struggle to convert research into everyday practice that reduces exposure, improves early detection and embeds long-term behaviour change.

Education, practical intervention and credible collaboration have therefore become central to closing that gap.

Andy Slater, Managing Director for EMEA and Asia Pacific at PGI Safety, has focused on translating evidence into frontline action through education, PPE development and shared ownership.

Drawing on sustained engagement with firefighters, clinicians and industry peers, Andy has helped shape initiatives aimed at cancer prevention, early screening and cultural change.

IFSJ Editor Iain Hoey spoke with Andy about how education, culture and practical solutions can improve firefighter health outcomes.

You have described cancer as the most serious health threat facing the fire service today. What evidence and lived experience led you to that view?

Cancer is now recognised as the leading cause of line-of-duty deaths among firefighters, surpassing acute incidents like fires or structural collapse.

In the UK, studies show firefighters face significantly higher mortality from certain cancers, with prostate cancer up to 3.8 times higher, leukaemia 3.17 times and oesophageal 2.42 times than the general population, and overall cancer mortality 1.6 times higher.

Internationally, organisations such as the IAFF report that occupational cancer accounts for around 70 to 80 percent of line-of-duty deaths in recent years.

Lived experience reinforces this: I have heard from firefighters diagnosed after years of service, often linking symptoms to repeated exposure.

Collaborations with advocates like John Lord, who survived cancer and now champions awareness through seminars, and Robbie Burns, whose personal testimony has moved entire rooms, have shared stories of colleagues affected young or posthumously.

The WHO’s IARC reclassified occupational firefighting as carcinogenic to humans in 2022, and UK surveys showing elevated diagnoses, including in younger age groups, confirm this is a real and ongoing epidemic we can address through better protection and practices.

How do firefighters encounter carcinogenic exposure, and where can daily practice reduce risk most effectively?

Firefighters encounter carcinogens primarily through inhalation of smoke and particulates, dermal absorption via skin contact with soot and toxins, especially on the neck, groin and wrists, and ingestion from hand-to-mouth transfer or contaminated gear.

Modern fires increasingly involve plastics and synthetics that release toxic chemicals such as PAHs, benzene and PFAS, which linger on PPE, appliances and within stations.

The greatest opportunities lie in consistent day-to-day decontamination protocols: immediate post-incident hygiene, showering within the hour, on-site laundering, particulate-blocking hoods to reduce neck exposure, separating clean and dirty zones in stations and appliances, and avoiding eating or storing gear in contaminated areas.

Simple changes, such as not taking PPE home and adopting breathable barrier fabrics, can dramatically cut absorption.

At PGI, we focus on gear that balances protection with comfort to encourage consistent use, turning these practices into routine habits that save lives over the long term.

How have your education and knowledge-sharing initiatives evolved in practice?

Education started with raising basic awareness of contaminants and has developed into targeted, evidence-based seminars such as the Invisible Risk series.

Early sessions focused heavily on the “why”, including exposures and statistics, but feedback showed firefighters needed the “how”: practical decontamination steps, PPE selection guidance and station culture changes.

We refined the approach by incorporating real firefighter stories, scientific input from toxicology experts, interactive gear demonstrations and emphatic presentations by Robbie Burns.

Collaborations with peers such as John Lord and Robbie have broadened reach, attracting hundreds from fire services, regulators and suppliers.

What works best are actionable takeaways, such as clear post-incident hygiene protocols, combined with peer-led discussions that build buy-in.

Post-seminar engagement has been strong, with firefighters sharing stories of changed behaviours, deeper understanding of PPE requirements and proactive steps such as pursuing SNOMED codes.

Departments have adopted better practices that reduce cross-contamination and encourage earlier health checks.

How can SNOMED codes improve screening and earlier clinical intervention for firefighters?

SNOMED CT codes allow GPs and NHS systems to record a patient’s occupation as firefighter, using code 106382009, and occupational exposure risks, such as 16090571000119109.

This flags elevated cancer risk in electronic records and prompts clinicians to consider occupational factors during consultations.

In practice, this enables targeted screening, including earlier PSA testing, faster referrals for symptoms that might otherwise be dismissed and inclusion in enhanced monitoring.

After promoting SNOMED codes through seminars, several firefighters secured rapid PSA checks, leading to early-stage diagnoses in some unfortunate cases.

Catching cancer very early delivers far better outcomes.

We urge firefighters to request that their GP adds these codes.

It is a simple administrative step with significant impact, as better data leads directly to earlier intervention and improved outcomes.

How do mental and behavioural health challenges interact with cancer risk and long-term outcomes?

PTSD, shift-work stress and burnout are prevalent within the fire service and interact strongly with physical risks.

Chronic stress weakens the immune system, potentially accelerating disease progression, while behaviours such as poor sleep, smoking, alcohol use or skipping health checks compound exposure effects.

Stress can also lead to complacency in decontamination, with firefighters rushing post-incident routines or failing to change contaminated gear properly.

Over time, this creates a cycle where poorer mental health worsens physical outcomes and cancer diagnoses add psychological burden.

Addressing mental and physical wellbeing together helps reduce overall risk, as healthier firefighters are more vigilant about exposure and early detection.

Why do you view inclusion, particularly for women, as a practical safety issue?

Inclusion is a safety imperative.

Women firefighters face risks linked to ill-fitting PPE designed for male builds, which can increase exposure through gaps, and research suggests higher risks for certain cancers, including bladder.

Hormonal factors and reproductive health considerations add further complexity.

This focus also widens to other blue light responders, particularly police and ambulance staff working fire scenes, where correct PPE and RPE fit is critical.

Exclusionary cultures deter reporting concerns or seeking help, delaying interventions.

By prioritising inclusive gear, diverse voices in training and addressing gender-specific risks in seminars, protection improves for all.

I have been fortunate to have individuals such as Dany Cotton and Michelle O’Toole contributing to this important conversation around women’s health and inclusion within the fire service.

How have peers such as John Lord and Robbie Burns shaped this work?

Collaboration underpins credibility.

Firefighters remain exposed to hazardous fire residues despite policy changes, says London Assembly Report

Firefighters continue to face exposure to toxic contaminants

A London Assembly Fire Committee report warns that firefighters in London may be routinely exposed to hazardous substances released during fires.

The report Exposure to fire contaminants in London: A hidden, growing risk? was produced by the London Assembly Fire Committee.

The findings identify asbestos, heavy metals and persistent organic pollutants among contaminants present at incidents.

The Committee notes that definitive UK evidence on cancer causation is still emerging.

Research presented to the Committee shows higher cancer incidence and abnormal tumour markers among firefighters compared with the general population.

Fire Brigade Union commissioned research found that 4% of firefighters surveyed, around 1 in 25, reported a cancer diagnosis in a self-reported sample of more than 11,000 personnel.

Prevalence was highest among those aged 35 to 39, around three times the rate seen in the general population.

Between 10% and 15% of respondents were recorded as having abnormal tumour markers, compared with a control group drawn from the public.

Policy progress and recommendations for national action

The Committee concludes that the London Fire Brigade (LFB) Fire Contaminants Policy has shifted attitudes away from the “badge of honour” of dirty kit.

Implementation varies widely across stations, partly because of limitations in personal protective equipment availability and older station layouts.

The Committee urges the Government and the National Fire Chiefs Council to reduce firefighters’ exposure to fire contaminants.

Recommendations include creating a UK-wide exposure and health monitoring programme for firefighters.

A pilot of operational exposure monitoring in London is also proposed.

Further measures call for stronger training, assurance and compliance with fire contaminants policies, improved PPE supply and procurement planning, and greater emphasis on health promotion within occupational health services.

The Committee also asks the Industrial Injuries Advisory Council to review evidence on cancer risks linked to firefighting.

Chair of the London Assembly Fire Committee Zack Polanski AM said: “Fires don’t end when the flames are out as toxic contaminants can remain long afterward.

“Firefighters accept risk as part of the job, but exposure to toxic chemicals should not be an unavoidable consequence of protecting the public.

“While London Fire Brigade has made real progress, this report shows more needs to be done to understand and reduce the long-term health risks firefighters face.”

The report treats decontamination practices, station design and long-term health monitoring as closely connected areas for further action.

Lifestyle references criticised by the Fire Brigades Union

The Fire Brigades Union (FBU) has responded to the London Assembly Fire Committee report Exposure to fire contaminants in London: A hidden, growing risk?.

In its statement, the FBU said the report includes a recommendation referring to “positive lifestyle changes, including smoking cessation, physical fitness, diet, sleep hygiene, and alcohol awareness”.

Steve Wright, Fire Brigades Union general secretary, said: “Firefighters are dying younger and living with life-altering cancers and chronic illness.

“These are preventable occupational deaths, and this is a national scandal.

“The UK is decades behind other countries on this issue, and this report underlines the urgent need for basic, long-overdue measures: stronger prevention measures, regular health monitoring for all firefighters, and re-evaluation of compensation for firefighters’ occupational diseases.

“However, firefighters will rightly find the references to ‘lifestyle’ offensive.

“Firefighters are not getting sick because of personal choices – they are getting sick because they are being systematically exposed to toxic substances in the course of their work.

“When so many other countries and international bodies are doing the right thing and recognising the risks firefighters face, it is time for our own government to stop ignoring the evidence and act.”

Anna Stec, Professor of Fire Chemistry and Toxicity at the University of Lancashire, said: “While the report makes positive recommendations on the need for a UK-wide rollout of harmonised contamination exposure tracking and health monitoring for firefighters, the argument that firefighter cancers and diseases could be reduced through lifestyle changes is fallacious.

“Lifestyle factors influence health outcomes in any population, but they do not explain the elevated rates of cancer and disease seen in firefighters.

“Why should firefighters be expected to adopt such changes in order to compensate for the toxic environments they are routinely exposed to at work?”

Why the investigation was launched

The report states that firefighters are expected to accept risk as part of their work, but that exposure to fire contaminants represents an emerging and less visible risk, particularly in relation to cancer.

Evidence presented to the Committee indicates that firefighters in London are exposed to products of combustion at fire incidents, and that these exposures are broadly similar to those faced elsewhere in the UK.

Cumulative exposure in London

Witnesses told the Committee that, because of the high number of fires attended in London, there may be risks associated with cumulative exposure, even though London is otherwise comparable to other large metropolitan areas.

Evidence and uncertainty on cancer causation

The report notes that definitive UK evidence of direct causal links between firefighting and specific cancers is still developing, given the varied nature of exposures and the time it can take for related illnesses to emerge.

International evidence context

The Committee references the International Agency for Research on Cancer’s 2023 designation of occupational exposure as a firefighter as carcinogenic, while also noting that more UK-specific research is needed.

Why uncertainty should not delay action

The Committee argues that the absence of definitive causal proof should not prevent action, and that better assessment, measurement and tracking of exposure is needed.

Call for a national monitoring programme

The report recommends that central government, working with the National Fire Chiefs Council and devolved administrations, establish a UK-wide firefighter exposure and health monitoring programme with national standards for data collection and long-term surveillance.

Recommendation on compensation evidence review

The Committee recommends that the Industrial Injuries Advisory Council undertake a fresh review of the evidence on cancer risks linked to firefighting, in light of developments since its 2021 position paper.

Progress made by London Fire Brigade

The Committee recognises that London Fire Brigade (LFB) has taken several steps since publishing its Fire Contaminants Policy in 2022, and that this has led to a clear cultural shift away from viewing dirty kit as a “badge of honour”.

Key elements of the policy in practice

The report describes changes such as safe disrobe procedures, the use of designated zones within fire stations to reduce cross-contamination, and the expectation that firefighters should shower within an hour of exposure.

Changes to frontline practice

Witnesses and site visits indicated that firefighters are now more focused on decontamination at incidents and on preventing secondary exposure when returning to stations or fire engines.

Persistent practical barriers

The report identifies ongoing challenges with the availability of personal protective equipment, including shortages reported in 2025, which have affected implementation of the policy in some cases.

Financial and operational pressures

The Committee notes that increased cleaning and replacement of PPE are placing additional pressure on fire and rescue service budgets.

Station design as a structural constraint

The report finds that many older fire stations were not designed with modern contamination control in mind, making it difficult to separate clean and contaminated areas consistently.

Cross-contamination risks in existing stations

The Committee observed that, in some stations, contaminated equipment still has to pass through areas intended to remain clean because of building layout constraints.

Recommendations on future PPE contracts

The report recommends that LFB ensure its next PPE contract is designed to meet current and anticipated demands from fire contaminants policies, including more frequent cleaning, faster turnaround times and sufficient stock levels.

Recommendation on exposure monitoring in London

The Committee calls for LFB to develop a time-limited pilot exposure monitoring programme for operational firefighters, scoped in 2026/27 and launched in 2027/28, developed with academic and occupational health experts.

Existing health screening arrangements

The report records that London firefighters receive routine medicals every three years, including basic cardiorespiratory checks and an asbestos-specific element.

Limits of cancer screening

The Committee notes that comprehensive cancer screening for firefighters is complex, and that some cancers most strongly associated with firefighting, such as mesothelioma and bladder cancer, do not currently have safe, reliable population-level screening tests in routine NHS use.

Prevention and occupational health

The report recommends that LFB strengthen its prevention-focused approach by embedding proactive health promotion, including support around smoking cessation, fitness, diet, sleep and alcohol, within routine occupational health provision.

Regulatory context

The Committee records that the Health and Safety Executive considers the existing regulatory framework, including the Health and Safety at Work Act, COSHH regulations and Workplace Regulations, sufficient to reflect firefighting’s classification as a carcinogenic occupation.

Overall conclusion of the Committee

The Committee concludes that LFB’s Fire Contaminants Policy represents a substantial step forward, but that further national coordination, better exposure data, improved PPE systems and updated fire station design standards are needed to reduce long-term health risks for firefighters.

US Senate backs firefighter cancer PSOB expansion in National Defense Authorization Act

NDAA adds PSOB presumption for firefighter cancer

The National Defense Authorization Act (NDAA) passed by Congress includes statutory text titled the Honoring Our Fallen Heroes provision, which amends the federal Public Safety Officers’ Benefits programme.

The change is set out in Section 1098 of the bill and adds a new subsection on “exposure-related cancers” to the PSOB law.

For firefighters, the provision directly addresses occupational cancer linked to repeated exposure to carcinogens encountered during emergency response, training and post-incident activities.

The statutory presumption recognises firefighter cancer as a potential line-of-duty injury for PSOB purposes when the eligibility conditions set out in the law are met.

What the bill changes in law

The bill states that “the exposure of a public safety officer to a carcinogen shall be presumed to constitute a personal injury” sustained in the line of duty and resulting in death or permanent and total disability, if the statutory conditions are met.

The bill defines “carcinogen” as an agent classified by the International Agency for Research on Cancer (IARC) under Group 1 or Group 2A and “reasonably linked to an exposure-related cancer”.

Eligibility time limits in the statute

The presumption applies if the exposure occurred while the public safety officer was engaged in a line of duty action or activity.

The bill states the public safety officer must have begun serving “not fewer than 5 years before the date of the diagnosis” with an exposure-related cancer.

The bill states the officer must be diagnosed “not more than 15 years after the public safety officer’s last date of active service”.

The bill states the exposure-related cancer must “directly and proximately” result in death or permanent and total disability.

When the presumption does not apply

The bill includes an exception stating that the presumption “shall not apply” if competent medical evidence establishes the carcinogen exposure “was not a substantial contributing factor” in the death or disability.

Cancers listed in the bill

The bill defines “exposure-related cancer” as a list that includes bladder cancer, brain cancer, breast cancer, cervical cancer, colon cancer, colorectal cancer, esophageal cancer, kidney cancer, leukemia, lung cancer, malignant melanoma, mesothelioma, multiple myeloma, non-Hodgkins lymphoma, ovarian cancer, prostate cancer, skin cancer, stomach cancer, testicular cancer and thyroid cancer.

The definition also includes “any form of cancer that is considered a WTC-related health condition” under the World Trade Center Health Program statute.

The bill also includes “any form of cancer added” through future updates.

Updates, evidence base and petitions

The bill requires that “from time to time but not less frequently than once every 3 years” the Director must review the definition of exposure-related cancer and update it where appropriate.

The bill states updates must be based on competent medical evidence showing significant risk to public safety officers from engagement in public safety activities.

The bill lists examples of evidence sources that may inform updates, including recommendations, risk assessments and scientific studies by the National Institute for Occupational Safety and Health, the National Toxicology Program, the National Academies of Sciences, Engineering, and Medicine and the International Agency for Research on Cancer.

The bill also states that any person may petition the Director to add a form of cancer to the definition, with a 180-day referral requirement for petitions that satisfy the content standard described in the statute.

Backdated applicability and claim window

The bill states the amendment applies to claims predicated on the death of a public safety officer on or after 1 January 2020 where the death is the direct and proximate result of an exposure-related cancer.

The bill also states it applies to a disability claim filed on or after 1 January 2020 where the disability is the direct and proximate result of an exposure-related cancer.

The bill adds that an individual seeking to file a claim predicated on the amendment is allowed to do so “within 3 years of the date of enactment”.

IAFC welcomes inclusion in the NDAA

Fire Chief Trisha L. Wolford, President and Board Chair of the International Association of Fire Chiefs, said: “The IAFC is grateful to see the Honoring Our Fallen Heroes Act included in the NDAA and on its way to being signed into law.

“This long-overdue reform ensures that the families of fire, law enforcement, and EMS personnel who die or become disabled from occupational cancer receive the federal benefits they deserve.

“By including occupational cancers in the Public Safety Officers’ Benefits program, Congress is honoring the sacrifices made by our fallen heroes and standing firmly with the families they leave behind.

“We commend the bipartisan champions, including Representatives Mary Gay Scanlon and Carlos A. Gimenez, the bill’s House sponsors, and Senators Amy Klobuchar and Kevin Cramer, the bill’s Senate sponsors, who advanced this critical measure and thank lawmakers in both chambers for their commitment to our nation’s fire and EMS personnel.”

IAFF calls it a turning point for PSOB

General President Edward Kelly, International Association of Fire Fighters, said: “We know the devastation occupational cancer has brought to our profession.

“When a fire fighter dies from job-related cancer, that is a line-of-duty death – and it is long past time the federal government recognized that truth.

“Congress’s passage of the Honor Act marks a significant turning point.

“This legislation ensures our fallen are honored, and their families are not left behind.”

Kelly added: “This was a truly bipartisan effort, and I want to thank those lawmakers on Capitol Hill – both Republican and Democrat – who stood with us on this critical legislation.

“I also want to thank President Trump, whose support was crucial in passing what is easily one of the most impactful pieces of legislation in our union’s history.”

Relevance for fire and safety professionals

Fire and rescue leaders, occupational health teams and HR functions may want to brief staff on the statutory time limits, particularly the five-year minimum service requirement before diagnosis and the 15-year limit after the last date of active service.

Departments supporting bereaved families may want to check how the bill’s applicability language linked to 1 January 2020 interacts with the additional three-year filing window tied to the date of enactment.

Services with exposure recording systems may also want to review how the bill defines carcinogen and how the PSOB presumption can be rebutted through competent medical evidence, as this will shape the documentation expected in claims.

The group provides education on long-term health risks linked to exposure during operations, and offers direct support for affected personnel.

ZIEGLER said the Hamburg meeting included a presentation from FeuerKrebs on its programmes and outreach strategies.

The discussion also included mutual assessments of current hygiene practices and areas where vehicle design or station protocols could reduce exposure.

Both parties agreed that a holistic approach to health protection is required, extending beyond the fire scene.

Hygiene and vehicle-based solutions discussed

ZIEGLER presented its existing vehicle-integrated hygiene and decontamination systems during the meeting.

The company said its product teams demonstrated how interior layouts and materials can be adapted to improve post-incident safety.

The firm reported that potential system enhancements and future collaboration were considered during the exchange.

Design considerations for improving contaminant separation and on-site clean-up were highlighted.

Cervantes said: “This isn’t supposed to be my story. It still doesn’t feel real. My husband was a fire fighter for over 22 years. He worked for his dream department, got promoted to engineer medic, and even served as union vice president. We thought we had a lifetime ahead of us.”

She added: “Passing this would help support people like my hero who lost his life to cancer. We don’t want other families to go through what we did, without the support they need and deserve.”

The IAFF reported that Cervantes and her children visited congressional offices throughout the day to discuss the occupational cancer risks firefighters face.
