New report finds gaps in preparedness for public health emergencies

The COVID-19 pandemic has starkly demonstrated that underinvestment in preparedness to respond to public health emergencies can cost hundreds of thousands of lives and wreak havoc on the economy. A new report released today by Trust for America’s Health (TFAH) measured states’ performance on 10 key emergency preparedness indicators and found room for improvement in every jurisdiction.

The report, Ready or Not 2021: Protecting the Public’s Health from Diseases, Disasters and Bioterrorism measured states’ readiness for health emergencies of any kind (not just the COVID-19 crisis). It placed 20 states and the District of Columbia in a high level of preparedness category, 15 states in a middle preparation tier and 15 states in a low degree of preparation tier.

For nearly two decades, TFAH has examined the nation’s public health emergency preparedness in this annual report. During that time, all states have made improvements in their emergency preparedness, but all continue to have room for additional strengthening of their public health preparedness and emergency response.

“The importance of this report is that it gives states actionable data to adopt policies that save lives. The COVID-19 crisis shows that we have much more work to do to protect Americans from health threats, particularly in the ways in which structural racism create and exacerbate health risks within communities of color,” said John Auerbach, President and CEO of Trust for America’s Health. “States need to take aggressive steps to shore up their preparedness for all types of public health emergencies.”

While the report’s findings are not a measure of any state’s COVID-19 response, they demonstrate that while states’ readiness is important, national health emergencies on the scale of a pandemic require strong federal leadership and coordination, and long-term investment in public health infrastructure and workforce. States alone, even those that rank high in this report, are not sufficiently equipped to respond to a pandemic without federal help, say the report’s authors.

The report found:

A majority of states have made preparations to expand healthcare and public health capabilities in an emergency, often through collaboration. Thirty-four states participated in the Nurse Licensure Compact, up from 26 in 2017. The compact allows registered nurses and licensed practical or vocational nurses to practice in multiple jurisdictions with a single license.

In an emergency, this enables health officials to quickly increase their staffing levels. In addition, hospitals in most states have a high degree of participation in healthcare coalitions. On average, 89 percent of hospitals were in a coalition and 17 states and the District of Columbia had universal participation, meaning every hospital in the jurisdiction was part of a coalition. Such coalitions bring hospitals and other healthcare facilities together with emergency management and public health officials to plan for and respond to incidents. Every state and the District of Columbia had public health laboratories that had plans for a large influx of testing needs. This ability to surge laboratory testing capacity during the COVID-19 crisis was extremely critical.

Most states are accredited in the areas of public health, emergency management, or both. As of December 2020, the Public Health Accreditation Board (PHAB) or the Emergency Management Accreditation Program (EMAP) accredited 42 states and the District of Columbia; 29 states and the District of Columbia were accredited by both groups, a net increase of one since November 2019. Both programs help ensure that necessary emergency preparation and response systems are in place and staffed by qualified personnel.

Seasonal flu vaccination rates, while still too low, have risen significantly. The seasonal flu vaccination rate among Americans ages 6 months or older rose from 42 percent during the 2017–2018 season to 52 percent during the 2019–2020 season but is still below the 70 percent target vaccination rate set by Healthy People 2030.  

In 2019, only 55 percent of employed state residents, on average, used paid time off, the same percentage as in 2018. Those without paid leave are more likely to work when they are sick and risk spreading infection. This became particularly relevant during the COVID-19 pandemic, as isolation and quarantine are important tools for controlling the outbreak.

Most residents who got their household water through a community water system had access to safe water. On average, just 5 percent of state residents used a community water system in 2019 that did not meet all applicable health-based standards.

The report includes recommendations for actions by federal and state policymakers to improve the nation’s public health emergency preparedness in seven priority areas:

  • Provide stable, sufficient funding for domestic and global public health security.
  • Strengthen policies and systems to prevent and respond to outbreaks and pandemics.
  • Build resilient communities and promote health equity generally and in preparedness.
  • Ensure effective public health leadership, coordination, and workforce.
  • Accelerate development and distribution, including last mile distribution, of medical countermeasures.
  • Strengthen the healthcare system’s ability to respond and recover during and from health emergencies.
  • Prepare for environmental threats and extreme weather.

Honeywell sign agreement to acquire Fiplex Communications

Honeywell has signed an agreement to acquire a majority stake in Fiplex Communications, Inc., a Miami-based company that develops in-building communications systems, including bi-directional amplifiers (BDAs) that provide continuous and critical in-building radio coverage in challenging environments to improve the safety of first responders and building occupants.

The acquisition will expand the in-building connectivity and communications solutions that Honeywell provides to its customers. Fiplex’s solutions will become a platform of innovation for Honeywell wireless technologies.

The building communications industry is a fast-growing segment, largely driven by the adoption of enhanced fire codes. Its growth is expected to accelerate with the adoption of advanced technologies that will continue to change how building data is exchanged with various stakeholders.

“We are looking to reshape how communications and connectivity within buildings work. The addition of Fiplex’s products and software allows us to move toward creating the next generation of fire and life safety solutions to keep both building occupants and first responders safer,” said Vimal Kapur, president and chief executive officer, Honeywell Building Technologies. “The Fiplex team brings superior engineering talent and unique capabilities for wireless communications that will enable us to enhance our in-building communications offerings and complement our innovative, industry-leading solutions.”

According to a 2017 International Association of Fire Chiefs (IAFC) survey by the Safer Buildings Coalition, 56% of fire departments have experienced a communications failure in a building during an emergency event, leaving firefighters and other first responders virtually stranded in these situations after losing communications. In-building communications systems, like those from Fiplex, enable first responders to stay connected throughout a building that may have a poor cellular signal due to Low-E glass windows, energy-saving cladding or wall composition, and even in underground areas such as car parks. In-building communications can also support occupant safety with timely, clear and accurate notifications of emergencies and recommended responses.

Terms of the investment were not disclosed, and there is no change to Honeywell’s financial outlook as a result of the acquisition. The acquisition is pending customary deal closing conditions.

US emergency responders have mixed feelings on COVID vaccine

A study has shown that many US emergency responders have mixed feelings on the COVID-19 vaccine. Of the 3,169 respondents to the survey, more than half of the first responders who replied were uncertain about or reported low acceptance of the vaccine.

US emergency responders, including Firefighters and emergency medical services workers are at high risk of exposure to COVID-19 while on the job and pose an additional risk of transmitting the virus to others. Although vaccines are a promising public health tool for reducing COVID-19 transmission, little has been known about the perceptions of the COVID-19 vaccine among first responders.

To provide insight, a University of Miami Miller School of Medicine-led study queried a national sample of U.S. firefighters and emergency medical services workers through an anonymous online survey.

“Through the national sample of firefighters and emergency medical services workers, we gained insight into the workforce’s hesitancy about the COVID-19 vaccine,” said study lead and senior author Alberto J. Caban-Martinez, D.O., Ph.D., M.P.H., associate professor of public health sciences in the Division of Environment & Public Health at the Miller School. “We can leverage this study’s information to design workplace interventions that educate and encourage our first responders to receive the COVID-19 vaccine.”

Demographics determine perceptions

Of the 3,169 respondents to the survey, 48.2% expressed high acceptability of the COVID-19 vaccine, 24.2% were unsure, and 27.6% reported low acceptability. The results also revealed key demographic characteristics — such as age, race, ethnicity, education, marital status, and job ranking — for each group of respondents.

Additionally, across all ten geographic regions of the Federal Emergency Management Agency, the southeast (43.1%), the southwest (32.7%) and the west (34.1%) had the highest proportion of first responders who showed low COVID-19 vaccine acceptability.

“An important predictor we discovered from our study was that first responders who had not reported receipt of the influenza vaccine in the prior season had higher odds of being unsure about or not wanting to receive the COVID-19 vaccine,” Dr. Caban-Martinez said.

In the study, the co-authors note the importance of tailoring public health campaigns for educating those sub-groups of firefighters and emergency medical service workers who identified as unsure or expressed low COVID-19 acceptability.

The study results were published online Feb. 1 in the Journal of Occupational and Environmental Medicine.

Safety Stand Down 2021 to focus on “Rebuild Rehab” for first responders

Firefighting puts intense strain on firefighters, both physically and mentally. It is critical that firefighters receive a comprehensive rehab program after fire incidents or intense training to ensure their physiological and psychological wellbeing. To address this issue, the theme of the 2021 Safety Stand Down campaign is “Rebuild Rehab.”

Sponsored by the International Association of Fire Chiefs (IAFC) Safety, Health and Survival Section, the National Volunteer Fire Council (NVFC), and the National Fire Protection Association (NFPA), this year’s Safety Stand Down will take place June 20-26 and call attention to the need for physical and psychological rehab.

Rehab frequently does not get the consideration it deserves. Many think simply providing food and beverages to firefighters constitutes a rehab program. The 2021 Safety Stand Down challenges departments to reframe their thinking around rehab to mitigate all of the impacts of firefighting and make sure everyone is ready to respond to the next emergency. Rehab should encompass all areas of post-incident health, including cardiac, nutrition, exposure, mental health, hydration, and heat stress.

Responder agencies are encouraged to suspend all non-emergency activities during the week of June 20-26 in order to focus on training and education related to this year’s theme. An entire week has been scheduled to ensure that all shifts and personnel can participate and access the topical information, training, and resources that are available at www.safetystanddown.org. The site will be updated periodically with new tools and resources leading up to Safety Stand Down so that departments can plan their education and awareness activities.

The NFPA will once again host the Fire Service Safety Stand Down Quiz to raise awareness and foster a greater understanding of this year’s theme. Everyone who completes the online quiz will be automatically entered into a sweepstakes; 200 randomly selected participants will win a commemorative Safety Stand Down challenge coin.

The successful Mars Rover landing, a mission that has been years in the making was celebrated worldwide yesterday as a landmark day for science. Joining the celebrations were W. L. As part of the successful Mars Rover Landing, Gore’s wire and cable products were critical components for successful data processing and communications between the Perseverance Rover and the descent stage of the mission. They assisted with the precise landing operation to the Martian surface until the Sky Crane enabling this operation departs. The intense entry, descent and landing (EDL) phase began when the spacecraft reached the top of the Martian atmosphere, travelling at about 12,100 mph (19,500 kph). EDL ended about seven minutes later with the rover stationary on the Martian surface.

Gore’s reliable products were selected for the mission because of their durable constructions that have been proven and trusted over time to withstand the harshest environments encountered in space. The same cables are also used on the actual rover, which is about the size of a car, but at 2,260 pounds (1,025 kilograms) it weighs significantly less. The mission is expected to last at least one Mars year, roughly 687 Earth days.

Gas detection by Teledyne leads fight against COVID

Teledyne Gas & Flame Detection is taking the global fight against COVID-19 to new levels, using its class leading gas detection and monitoring technologies to not only support vaccination programmes, but prevent the potential for virus propagation.

Gas detection can play a major role in the deployment of the Pfizer-BioNTech vaccine, which is manufactured in locations including the US, Germany and Belgium. This vaccine requires storage and transportation at -70°C in sealed containers due to its mRNA (Messenger RNA) molecule technology.

Such a low temperature is maintained using dry ice. Dry ice is solid, frozen carbon dioxide (CO2). However, at temperatures above -78°C, CO2 sublimates, which is when it becomes gaseous and potentially dangerous. ‘Red Flag Warning’ – high winds further spread US West Coast wildfires

The US National Weather Service has issued a “red flag warning” for areas of the West Coast, as high winds threaten to spread devastating wildfires still further.

Gusts of up to 40 mph (64 km/h) are expected in parts of southern Oregon, with no rain.

Fires have been raging for three weeks in Oregon, California and Washington, burning land and destroying homes.

Tens of thousands of people have had to flee and at least 33 have died.

Dozens of people are missing in Oregon alone, with one emergency official saying the state should be preparing for a “mass fatality incident”.

Cooler, moister conditions on Saturday allowed firefighters to make some progress with the fires.

However, the forecast for Sunday prompted a warning of winds of 15-25 mph and gusts of 30-40 mph in the Shasta Valley and east of the Southern Oregon cascades.

A red flag warning is the highest alert for events which could result in major fires, and which demands extreme caution by residents.

This area includes Jackson County, Oregon, where the Almeda fire has destroyed hundreds of homes and remains only 50% contained.

The fires have so far scorched an area of land the size of New Jersey, officials say.

The smoke pollution from the wildfires has left Oregon’s largest city, Portland, with the worst air quality in the world, followed by San Francisco and Seattle, according to recent findings from IQAir.

Helicopter pilot killed in California wildfires

A helicopter pilot involved in fighting fires in California died yesterday (19th August) when the aircraft crashed near Coalinga, authorities said.

According to Federal Aviation Administration spokesperson Ian Gregory, the Bell UH-1 helicopter was helping to fight the Hills Fire when it crashed around 10 a.m., about 11 miles south of New Coalinga Municipal Airport, the Fresno Bee reported.

In a statement, Cal Fire said the helicopter crashed while it was on a water-dropping mission on the Hills Fire in western Fresno County. The blaze has been burning in the area since Saturday and has consumed 1,500 acres and is 35% contained, KGO reported. Winds of 26 mph have been clocked in the area, with gusts to 35 mph.

The impact of the helicopter crash ignited a new brush fire, Gregory told reporters.

The pilot, who was not identified, was the only person aboard the helicopter, according to Cal Fire officials.

“There was one person on board, they did not survive,” Cal Fire spokesman Daniel Berlant told the Los Angeles Times.

Fresno County Sheriff’s Office spokesman Tony Botti said the department’s search and rescue team assisted in the search for survivors, the Bee reported.

The FAA and National Transportation Safety Board will investigate, with the NTSB in charge of determining the probable cause of the crash, the newspaper reported.

According to fire officials, 50 structures have burned down in Vacaville, with another 50 damaged by the fire.

“We are experiencing fires the likes of which we haven’t seen in many, many years,” Governor Newsom told reporters.

The blazes are thought to have been sparked by a heatwave combined with nearly 11,000 lightning strikes which have hit the state over the last three days.

Earlier this week, one of the highest ever temperatures on Earth was recorded in California’s Death Valley. Across the US western states on Wednesday, nearly 45 million people were living in areas under some form of excessive heat warning or heat advisory.

Mr Newsom said 367 known fires have been recorded so far, but noted that “the prospect of that going up is very real”.

“As those lightning strikes spark, as you have a lot of smoke, you have a difficult time determining total number of fires until certain things clear and we have the opportunity to go to more remote parts of the state,” he added.

The group of fires in the San Francisco Bay Area was driven by winds throughout Tuesday night and has now grown to 46,000 uncontained acres.

The fire, which is larger than city of Washington DC, has injured four people and threatens about 2,000 buildings in Vacaville, authorities say.
